RFAM offers a customized screening form that match regulations for facility staff, the public and corporate visitor.
Users receive a unique QR code along with a 1 pager for facility entrance points.
This module grabs the list of facilities and open spaces from the inventory module so staff can monitor facility/open space traffic.
Once filled out a recap of their response is displayed for facility attendants to verify before admitting into the facility - confirming all individuals are filling in the form and do not have any COVID-19 symptoms.

Track individuals entering your facility and make sure no one has a COVID-19 symptom. Contact those that may have come in touch with an individual that tested positive for COVID-19. In addition, data is automatically deleted after 30 days saving paper & time every single day.
